Chennai: Radhika got the first dose of the Covishield vaccine in Alwarpet in June, but did not get its certificate.
Now, Portal Cowin said the 21-year-old child was not vaccinated at all.
Some other people are in the same situation.
Someone must be vaccinated the third time because the staff at the Greater Chennai corporate center did not enter the details into the portal while giving the first dose.
Others were given a Covishield certificate after taking Covaxin’s shot.
The Civic body has received at least 50 complaints like that almost every day since the vaccination drive starts, an official said.

The lack of trained data entry operators, problems with networks and Wi-Fi, website traffic and lack of adequate staff.
“Even if the website doesn’t work, the staff might ask the person to be vaccinated and maybe forget to upload details on the portal later.
Sometimes, they are not registered at all.
And the wrong entries by staff often occur,” said a corporate official, added that efforts are made to try and help them with complaints.
Residents say there must be a general portal or certain complaint mechanism.
Umanath V of Mogappair completed both vaccination doses, but the portal did not receive the second dose because the age was inserted wrong.
“I was asked to go to the Ripon building for correction.
Why are we faced with very troublesome because of their mistakes,” he asked.
Bharath Ragnanathan from Kodambakkam got a Covaxin shot with his Aadhaar card but the Covishield certificate and asked the Pan card to change it.
In accordance with the Cowin portal, until September 3, 50.8 lakh people in Chennai have been vaccinated so far, but corporate figures are 40.4 lakh.
The wrong entry like that will not affect the actual number, said Vice Commissioner (Revenue and Finance) Vishu Mahajan, adding that the Cowin portal is only for registration and while the actual amount is inserted on the e-Win portal.
“Our numbers may not match the Cowin number until it is reconciled properly,” he said.
A health department officer said residents could make mistakes repaired in vaccination centers in question.
“The country helps us because we send details of all the entries and corrections wrong every day.”
